Together for Chester is committed to improving healthcare access for Chester residents by connecting them with Health Ambassadors and Community Health Workers. These trusted advocates, located in familiar local venues, serve as vital bridges to primary care providers, specialty care, and essential social services. Through collaboration and resource mobilization, we strive to address the social determinants of health, empowering the Chester community to achieve equitable and comprehensive well-being.
Together for Chester envisions a thriving community where every resident has equitable access to high-quality healthcare and social services. By leveraging trusted partnerships and mobilizing existing resources, we aim to create a healthier Chester, where all individuals feel supported and connected to the care they need.
The community of Chester has encountered significant challenges in accessing healthcare, mainly due to socioeconomic barriers, limited resources, and health disparities. Addressing these issues demands innovative solutions that leverage the expertise and resources of various organizations. Together for Chester is a collaboration that includes healthcare providers, government leaders, community leaders, nonprofit organizations, and academic institutions. Through this partnership, we have developed and implemented impactful initiatives aimed at tackling the healthcare challenges faced by the Chester community.
You should visit your doctor regularly, even if you feel fine. These visits help to find and prevent serious health problems that might not show clear signs at first. Your PCP is the best person to go to for routine health care. You can also see them for non-emergency health issues, such as reproductive health, eating habits, sleep troubles, managing stress, mental health, or minor sicknesses and injuries.

Health Ambassadors (HA) are community members or individuals within an organization who are trained to promote health and well-being. They often serve as peer educators and resources for others. Health Ambassadors can take your information and connect you with a Community Health Worker.
Community Health Workers (CHWs) serve as bridges between communities and healthcare providers. They assist individuals by educating them about health issues, navigating the healthcare system, and connecting them to local resources and transportation options. Additionally, CHWs can help complete public health insurance applications, locate healthcare providers, and schedule healthcare appointments.
Trusted venues are places in the community where trained individuals, called Health Ambassadors, share relevant information about health and other helpful resources available nearby.

The Foundation for Delaware County offers community programs that aim to improve birth outcomes, maternal and child health, and family overall well-being. These programs include Healthy Start, the Nurse-Family Partnership, the WIC Clinic, Cribs for Kids, the Women and Children's Program, and Hope (Housing Opportunities Program for Equity).
The Foundation for Delaware County supports expecting mothers, women experiencing postpartum challenges, new parents, caregivers of young children, and pediatric care.
